Read this as a repositioning file, not as a second sildenafil monograph. The bench in Sandwich, Kent, spent the late 1980s on pyrazolopyrimidine PDE5 inhibitors because coronary smooth muscle looked like a target. Erectile tissue was not the brief.
UK-92,480 - also written UK-92480 in some ledgers - was synthesised in 1989. Nature Reviews Drug Discovery later retold the arc: healthy-volunteer work from 1991, intravenous looks in angina, a glyceryl-trinitrate interaction study, then a hard pivot when the chest indication thinned and the erection reports did not.
What we bind here is the leftover counseling. A 25 mg diamond is the smallest labeled tablet and the start this site uses when exposure is likely to run high. An empty plate is not folklore. It is PK that an angina team measured because they cared about onset and peak.

Mid-1980s Pfizer Europe was after new cardiovascular levers. PDE5 looked promising because it breaks down cGMP, the same messenger nitric oxide uses to relax vessels. A selective inhibitor might ease angina without the mess of a cruder vasodilator.
By 1986 the pyrazolopyrimidine series was already potent on PDE5. UK-92,480 earned the development slot on potency and a cleaner spread versus PDE1 through PDE4. PDE6 in the retina was the near-neighbour nobody wanted to advertise yet.
Ian Osterloh's early clinical work - often tied to Morriston Hospital in Swansea - gave single oral doses up to 200 mg. The plasma half-life sat near four hours. Headache and flushing showed up as expected for a vasodilator. At 150 mg and above, some men reported a transient colour-vision shift. That was PDE6 talking, not a party trick.
Food as the angina programme's leftover
High-fat meals slow the rate of absorption. Mean Tmax slips by about 60 minutes. Mean Cmax drops about 29 percent. Median Tmax on an empty stomach sits near 60 minutes (range 30 to 120). That is why this folio still says: if you want the peak on time, clear the plate.
The angina team cared about onset because they were scoring hemodynamics, not date nights. The leftover counseling is the same curve. A cheeseburger does not 'turn the diamond off.' It makes a punctual 50-minute plan into a late, softer peak.

A vasodilator born on an angina bench taught the company to respect peak and load. Starting at the smallest scored diamond is that lesson, not a claim that 25 mg is 'the usual dose for everyone.' If 25 mg is a blank after a fair trial on an empty plate, the prescriber can climb. Climbing past a dirty hold is not an option.
Once-daily frequency is the ceiling on the ED label. A second diamond the same calendar day is not a 'boost.' It is off the slip. The four-hour half-life tempts people to redose because the first attempt felt late after a meal. Fix the plate or the clock. Do not invent a twice-a-day Viagra habit.
What the origin file still forbids
Nitrates and riociguat remain shut. The GTN study was not a footnote. It was the reason the chest programme died and the reason a diamond still cannot sit next to a spray.
Sudden vision loss, sudden hearing loss, and an erection beyond four hours are stop-and-seek-care marks on every PDE5, including this first one. Colour wash that fades as the four-hour half-life clears is the old PDE6 finding at a smaller dose. A blacked-out eye is not that finding.
Sex is work for a heart. If a clinician has already said the heart cannot take that work, the origin story does not overrule them. Bind the hold. Then, if the hold is clean, a 25 mg first diamond on a cleared plate is how this shelf starts the file.
Revatio and other pulmonary-hypertension sildenafil marks are a different indication and a different daily load. Do not borrow those milligrams to 'boost' a 25 mg ED diamond. The origin file is about the first oral ED stamp in 1998, not about stacking two sildenafil products because a forum treated them as cousins you can mix.
Why the chest-pain file stalled
Intravenous UK-92,480 in people with angina gave only a moderate vasodilatory nudge. That is a weak offer when the market already has nitrates, beta-blockers, and calcium-channel drugs with harder outcome data.
Worse: a volunteer study built to test a glyceryl-trinitrate interaction showed the compound amplified GTN's fall in blood pressure. For a would-be angina tablet, that is close to a fatal plot hole. The men most likely to need a chest drug are the men most likely to carry a nitrate.
So the angina file did not 'almost win.' It ran into a pharmacologic wall the label still prints: sildenafil plus any nitric-oxide donor is contraindicated. The miss is the origin of today's hold, not a colourful anecdote.
The leftover that became a first diamond
Volunteer diaries kept returning an effect the angina protocol had not ranked: penile erection. Nurses collecting leftover tablets after a Welsh chest study noticed men were unusually unwilling to give the bottles back. That story has been retold until it sounds like folklore. The pharmacology under it is ordinary: PDE5 sits in cavernosal muscle as well as in vessels.
Pfizer flipped the indication. Between 1993 and 1997 the company ran an ED programme large enough that, by 1997, more than 4,500 people had seen the drug across 21 trials. Efficacy in men with ED was no longer a rumour. Side effects stayed the vasodilator set: headache, flush, dyspepsia, and dose-linked colour shift.
European approval followed in September 1998. The diamond became a cultural object faster than most tablets ever do. That fame is not a counseling tool. The useful leftover is still the hold the angina team measured and the 25 mg caution start for high-AUC bodies. A famous shape does not make a dirty nitrate line safer.
March 1998 made it the first oral ED tablet the FDA would stamp. Injections and vacuum devices already existed. A swallowable diamond changed who would walk into a clinic. It did not retire the nitrate rule the angina team had already proved.
History is a hold, not folklore
Draw the Sandwich year. Note that UK-92,480 missed angina because the nitrate collision was real and the chest signal was thin. Shelf-check today's list for the same collision, for ritonavir, for age and kidneys. Bind a 25 mg first diamond only after that check.
The leftover meal rule is still the leftover meal rule. Empty plate if the clock matters. Fatty dinner if you can wait. Neither choice lifts a dirty nitrate line.

So a plan that assumed 'ready at 70 minutes' becomes a later, softer peak. The diamond is not cancelled. The clock is. If the evening is a heavy meal, either swallow earlier or accept a slower onset. Colour wash showed up at 150 mg in early files. Does 25 mg still do that?
Early volunteer work flagged transient colour-vision change at 150 mg and above, later tied to weak PDE6 hit in photoreceptors. At 25 mg the same mechanism can still tinge blue or brighten lights in a minority, usually while the four-hour half-life is still in the blood, then fade. That is not the same event as sudden vision loss in one eye, which means stop the drug and seek urgent care. Ritonavir and a 25 mg diamond - is 48 hours the real gap?
Yes, on the Viagra label: do not exceed a single 25 mg dose of sildenafil in a 48-hour period with ritonavir, because the AUC jump in the interaction study was about eleven-fold. That is not a 'maybe stretch it' rule. If ritonavir sits on the list, the first diamond is also the ceiling until an HIV clinician and the prescriber share the same page. Do not borrow a 50 mg or 100 mg habit from a friend. The angina team already taught us that peak load is not theoretical.
Empty plate - folklore, or leftover PK from the chest programme?
Leftover PK. Fasted Tmax is quicker. High-fat Tmax is later and Cmax is lower by about a third. The angina group measured food because they were watching onset of a vasodilator. We kept the sentence because couples still time an evening. Folklore would be 'never eat again.' The accurate bind is: food is allowed; fat delays the peak. Chest pain after a diamond - what do I tell the ambulance?
Tell them the hour you swallowed sildenafil and the milligram. Nitrates are a usual emergency reflex for ischemic pain, and they are contraindicated after a PDE5 inhibitor.
